CaseCharles is a 21-year-old single Caucasian man currently residing with his mother and stepfather who is also Charles' uncle. Charles graduated from high school and, due to his illness, receives Social Security benefits. Over a two-year period, Charles had nine emergency room visits that resulted in his admission to the psychiatric unit. Of two admissions Charles left against medical advice, five admissions required a higher level of care resulting in admission to the state psychiatric hospital and two Charles was transferred to the adult crisis unit. Charles also has a history of crimes, primarily public nuisance due to drug addiction, primarily marijuana and cocaine. Charles was evicted after a psychotic episode and the destruction of his apartment. Charles agreed to the medication protocol including Haldol and Resperadol injections. He categorically refuses psychotherapy. While hospitalized, Charles makes reference to sexual abuse, but refuses to go into detail or give details. Before the diagnosis, Charles' mother's reports were withdrawn when he was seven years old. Charles' father died in a car accident. At the age of twenty, Charles experienced his first psychotic attack. Not knowing what was happening, his mother called the police because she believed the hallucinations and delusions were due to marijuana and cocaine use. Charles was taken to the local jail and then hospital when symptoms persisted for 12 hours. At that time, Charles was diagnosed with schizophrenia. The mother cannot understand the diagnosis and believes that the etiology of the psychosis is solely due to drug use. Charles is also in denial but accepts the medication and when he feels better he stops and resumes his drug addiction.
