    Analysis of the Smoking Guns MemosPart I: IntroductionTwo tragic incidents, the crash of the Space Shuttle Challenger in 1986 and the near collapse of Three Mile Island in 1979, greatly affected devastated our nation. Both disasters were due to communication failures among ordinary professionals working in largely bureaucratized businesses. Two memos called "Smoking Gun Memos", written by RM Boisjoly, of Morton Thiokol, and DF Hallman, of Babcook and Wilcox, will always be associated with these two incidents. Unfortunately, none of these memos succeeded in preventing the Challenger and near-meltdown Three Mile Island accidents.RM Boisjoly:RM Boisjoly had more than a quarter century of experience in the aerospace industry in 1985 when he became involved in an effort to improve the O-ring that connects the segments of Morton Thiokol's Solid Rocket Booster. This was used to put the Space Shuttle into orbit (OEC, 2006). Morton Thiokol is an aerospace company that manufactures the solid propellant rocket engines used to launch the Challenger (Skubik). Boisjoly wrote a memo to RL Lund, vice president of engineering, and four others regarding his concerns about the faulty O-ring erosion problem. His warnings were ignored, leading to the deaths of six astronauts and a social studies teacher. DF Hallman: DF Hallman was Director of Plant Performance Services at Babcock and Wilson. The Babcock and Wilson Company, founded in 1867, manufactured and sold specialized industrial products, including fossil fuel and nuclear systems (Gorinson and Kane, 1979).
